A referral form is submitted through our 'Patient Referral Form.' Once the form has been received, we will get to work assigning a provider! To expedite your process, please make sure to include all necessary information such as wound photos, H&P, demographics, insurance, etc. Allow at least 2 business days to complete this step
We will update you every step of the way. You can choose to be called, texted, or emailed when there is an update. A new technology is being developed to enable you to log in to a portal and see live updates as the providers complete tasks.
We follow each referral until a care plan is in progress with the provider. Our patient status updates will be given when:
* Provider has been assigned
* Patient has been contacted
* 1st appointment is scheduled
* 1st appointment is completed
* Visit notes and care plan available
The more detailed the information provided, the easier it is for us to understand the patient’s needs and ensure the best possible care. Typically, our providers require:
Basic Patient Information (e.g., face sheet, patient location, phone number)
Wound Photo(s) & Basic Wound Information
Insurance Information
History & Physicals
Additionally, providers may request:

We aim to provide timely care for all referred patients. While we try to follow these general timeframes, cases in rural or hard-to-staff areas may occasionally take longer.
Patient Accepted & Contacted: Typically within 48 hours of receiving the referral.
First Appointment Scheduled: Typically within 72 hours of receiving the referral.
Visit Notes & Care Plan Available: Typically within 48 hours after the visit.
Follow-up Visit Scheduled: Updated promptly once confirmed, with the date and time submitted along with visit notes.
Ongoing Care Updates: For patients receiving continuous care, weekly appointment schedules are included in the Wednesday update.
Yes! Almost all of our providers are mobile and able to travel to the patient's location.
For new patients, updates are provided more frequently to ensure a smooth start to care. You can expect the following updates:
When the patient is accepted and contacted – Typically within 48 hours of receiving the referral.

After each visit – Visit notes and care plans are typically available within 48 hours post-visit.
When a follow-up appointment is scheduled – This information is updated promptly once confirmed and submitted with visit notes.
For patients receiving ongoing care, we provide weekly updates every Wednesday to referrers and facilities. These updates include scheduled appointments for the upcoming week.
If a patient is in a rural or hard-to-staff area, there may be occasional delays, but we will keep you informed of any changes.
We work with a large selection of providers, allowing us to accept most insurance plans. However, Medicare is the most commonly accepted and easiest to process. If you have questions about a specific insurance provider or coverage details, please contact us, and we’ll be happy to assist you.
